Clinico-pathophysiological considerations in coronary microvascular disorders

چکیده

Around half of the patients undergoing an elective coronary angiogram to investigate typical stable angina symptoms are found have non-obstructive arteries (defined as < 50% stenosis). These younger with a female predilection. While underlying mechanisms responsible for these presentations heterogeneous, structural and functional abnormalities microvasculature highly prevalent. Thus, microvascular dysfunction (CMD) is increasingly recognised important consideration in arteries. This review will focus on primary disorders summarise four common clinical presentation pictures which can be considered endotypes - Microvascular Ischaemia (formerly “Syndrome X”), Angina, Spasm, Coronary Slow Flow. Furthermore, pathophysiological associated CMD also heterogenous. may arise from increased resistance, impaired dilation, and/or inducible spasm, ultimately causing myocardial ischaemia angina. Alternatively, chest pain hypersensitivity receptors rather than ischaemia. two major should when assessing individual picture, ultimately, question arises whether target heart or perception treat anginal symptoms.

Corticobasal degeneration -- clinico-pathological considerations.

Corticobasal degeneration (CBD) is a rare sporadic 4-repeat tauopathy. We report here the first Polish case of pathologically proven CBD. Our patient developed clumsiness of the right hand at age 63 years. During the course of his illness he suffered from progressive asymmetric parkinsonism unresponsive to dopaminergic therapy. Focal dystonia affecting right upper extremity, non-fluent aphasia,...

Coronary microvascular reserve and outcome in aortic stenosis: Pathophysiological significance vs. clinical relevance.

Degenerative aortic stenosis (AS) is one of the most common valvular diseases in developed countries. AS is a progressive disease for which the only effective treatment is surgical or transcatheter aortic valve replacement (AVR). Patients with mild to moderate AS require no specific valve treatment, whereas AVR is indicated in patients with severe symptomatic AS. The main conundrum in the manag...

Functional digestive disorders – pathophysiological approach

In contrast to organic diseases due to structural lesions or humoral aberrations, functional disorders have been ascribed to abnormal functions. But which functions? The term Functional Digestive (or Gastrointestinal) Disorders was introduced after discovery of tests of gastric secretion at the end of the 19th century when digestive symptoms were ascribed to abnormal gastric acidity. The diagno...

How Many Types of Dystonia? Pathophysiological Considerations

Dystonia can be seen in a number of different phenotypes that may arise from different etiologies. The pathophysiological substrate of dystonia is related to three lines of research. The first postulate a loss of inhibition which may account for the excess of movement and for the overflow phenomena. A second abnormality is sensory dysfunction which is related to the mild sensory complaints in p...

Adrenal Disorders and the Paediatric Brain: Pathophysiological Considerations and Clinical Implications

Various neurological and psychiatric manifestations have been recorded in children with adrenal disorders. Based on literature review and on personal case-studies and case-series we focused on the pathophysiological and clinical implications of glucocorticoid-related, mineralcorticoid-related, and catecholamine-related paediatric nervous system involvement. Childhood Cushing syndrome can be ass...
